Coumadin, a blood-thinning medication, requires vigilant monitoring because the potential for food or drug interactions and side effects is high. Providing comforting, one-on-one support, ValleyCare Clinical Pharmacy staff works with patients to monitor blood tests, ensure adherence and proper dosage and check for possible complications.
