American College of Cardiology Platinum Performance Achievement Award, 2016
Stanford Health Care – ValleyCare consistently followed the treatment guidelines in ACTION Registry®–GWTG™ for eight consecutive quarters in ACTION Registry–GWTG Premier and met a performance standard of 90% for specific performance measures to receive this 2016 award. ACTION Registry–GWTG is a partnership between the American College of Cardiology Foundation and the American Heart Association with partnering support from the American College of Emergency Physicians and the Society of Cardiovascular Patient Care. Centers for Medicare & Medicaid Services 5-Star Quality Rating
The Centers for Medicare & Medicaid Services (CMS) awarded Stanford Health Care – ValleyCare's Skilled Nursing Facility a FIVE-STAR overall rating. The star ratings are derived from three performance measures including: state inspections, staffing levels and quality measures. Stanford Heath Care – ValleyCare scored above average in state and national averages for quality measures, which includes: functional rehabilitation assessment/improvement toward goals, pain management, and administering seasonal flu and pneumonia vaccines.
