ValleyCare Health System's Case Management and Social Services Department offers a comprehensive team of nurse case managers, social workers and discharge-planning assistants to facilitate safe, timely and coordinated discharges.
During your hospital stay our team will support you and your family members during this stressful time. We can give you resources and provide information for you to make the medical care decisions that are right for you and your family.
While you are still in the hospital, our patient care team is working with you and your family to identify the services you will need after discharge. Some of those services might be medical, educational, equipment and social or spiritual support needs. Working together we can arrange for home health services, skilled nursing facility or extended care facility placement and medical equipment to help support you and your family at home.
The Case Management/Social Services staff members are knowledgeable in Medicare, Medi-Cal, Healthy Families, California Children's Services and private insurance programs.
Advance Directives for Healthcare
An Advance Directive for Healthcare (ADHC) is a legal form that can be completed without a lawyer to make your healthcare wishes known when you can no longer communicate. It allows you to be in control of the care you receive while in the hospital and in other settings. The person you name as the agent is to carry out your wishes as stated in the ADHC. In this one form, you can name who you want to make healthcare decisions for you when you are unable to speak for yourself and define the types of treatment you would or would not like to receive.
