ValleyCare Health System offers inpatient and outpatient care for individuals with Type 1, 2 or gestational diabetes including individual counseling, group support, nutritional advice and education. The ValleyCare Diabetes Education Program has been Recognized by the American Diabetes Association for Quality Self-Management Education*.

Through our education programs you will learn skills to manage your diabetes. Education programs consist of the Living Well with Diabetes small group classes; individual education sessions and community education seminars. Skills learned include the following:
Healthy Eating: make healthy food choices, understand portion sizes and eat right for your lifestyle
Being Active: develop an activity plan for overall fitness, weight management and blood glucose control.
Monitoring: monitor blood glucose levels, blood pressure, cholesterol labwork and weight. Learn to interpret target values and how to use them effectively.
Problem Solving: sharpen problem-solving skills to make rapid, informed decisions about diet, activity and medications.
Reducing Risks: understand, seek and regularly obtain an array of preventive services to maximize health and quality of life.
Healthy Coping Skills: how to set achievable behavioral goals and work through obstacles, fears and doubts.
Living Well with Diabetes – Group Classes
In these interactive small group classes, you will learn to:
Enjoy eating using a personalized plan
Monitor your blood sugar
Control your hemoglobin A1C, cholesterol and triglycerides
Prevent/reduce complications
Enjoy eating using a personalized plan
Use your medications effectively
Manage stress and increase physical activity
At each class you will:
Have your blood pressure monitored and glucose tested
Learn how to problem solve and set goals
Receive professional expertise from Certified Diabetes Educators
A new class begins the second Wednesday of each month. The three-session class meets over the course two months for a total of 7.5 hours. Registration and physician referral are required. For session dates, call 925-416-6710.

Individual Diabetes Education Session
One-on-One education sessions are available to those who may require individualized diabetes education. This may include, but is not limited to, those individuals requiring follow-up after the group classes; or those with special needs such as: visual or hearing impairment, physical limitations or non-English speaking.

Affiliated with the California Diabetes and Pregnancy Program, The Sweet Success Program offers education and support for women diagnosed with gestational diabetes (high blood glucose levels detected during pregnancy), and those with existing diabetes who become pregnant. You will learn how to control glucose levels and reduce the complications of diabetes for you and your child, before, during and after pregnancy.
Pre-Conception Counseling
For women previously diagnosed with diabetes, counseling can begin when a couple is first considering pregnancy. Steps can be taken to reduce the risks associated with diabetes.
Pregnancy and Post Pregnancy
Our professionals can guide you through each phase of your pregnancy whether your glucose levels are managed by insulin, diet, exercise or a combination of these methods.
Following the birth of your child, our diabetes educators will continue to work with you to help you maintain a healthy weight and control your glucose levels.

Learn how to adjust insulin doses to fit your individual lifestyle, for flexible management of diabetes. Also learn skills required for successful blood glucose management using an insulin pump -- including advanced carbohydrate counting, insulin adjustment and safety protocols.

HODA GHANEM, MD
ENDOCRINOLOGY
Dr. Hoda Ghanem specializes in Endocrinology, Diabetes and Metabolism.
Dr. Ghanem is board certified in Internal Medicine. She is also a member of the Endocrine Society, American Association of Clinical Endocrinologists and the American Thyroid Association.
A native of the San Francisco Bay Area, Dr. Ghanem completed her undergraduate studies at the University of California, Berkeley, and received her medical degree from Temple University School of Medicine in Philadelphia, PA. She completed her internship and residency at the University of California, Irvine and an Endocrinology fellowship at Banner Good Samaritan Medical Center in Phoenix, AZ.
Her office is located at 5725 W. Las Positas Blvd. Suite 210, Pleasanton.

A doctor's referral is required for all of the services in the program. Equipment, supplies and educational health management costs for diabetes are covered by most health plans when deemed medically necessary by a physician. Medicare Part B covers Diabetes Self-Management Education.

All are welcome to attend our Diabetes Support Group, held on the first Wednesday of each month at 12:00 noon at the ValleyCare Medical Plaza, 5725 W. Las Positas, 2nd floor conference room, Pleasanton. No charge. No appointment necessary.
