For registration purposes, please bring your medical insurance cards, picture identification, social security number, copies of your Durable Power of Attorney for Health Care, a list of all current medications and the name and phone number of your closest relative.
Please limit your personal items to basic essentials (robe, slippers and toiletries). Leave or send all valuables (including jewelry) home with a family member.
Translation/TTY Services can be arranged by request prior to or at the time of admission. Over 130 languages are provided through the Communication and Language Line. |
